Send an intake form to a managed client

Managed (app-less) clients can fill out an intake form through a private link — no app required.

Managed clients don't have the app, but they can still complete an intake form. Cradled generates a private link they fill out in a browser, and the answers come straight back to you.

Sending a form

  1. Open the managed client and find the Forms & plans card.
  2. Click + Send form.
  3. In the Send a form via email dialog, pick a template.
  4. Choose how to deliver it:
    • Enter the client's email and click Send via email, or
    • Leave the email blank and click Just copy link to share it yourself (text, your own email — however you like).

The client opens the link (a cradledapp.com/form/<token> page), fills out the sections, and submits. Their responses appear back in the same Forms & plans card.

Tracking responses

Sent forms show their status — Sent, Opened, or Submitted (with a timestamp). You can copy the link again, or archive a form (which stops its link from working).

Connected clients

If a client is on the app, you don't need the email link — assign the plan directly and it shows up on their Care tab. See Assign a plan template.
