Birth plans, postpartum plans, intake forms

The three built-in plan types and what each is for.

Cradled has three core types of plan templates. Each one maps to a different moment in a client's journey.

Birth plan

What the client wants for her birth experience. Preferences around labour, pain management, interventions, environment (home, birth center, hospital), who she wants present, and more. Usually filled out in the third trimester.

Built for connected clients — they see sections on their Care tab and fill them out question by question. On the mom side, sections are marked with progress bars as she works through them.

Postpartum plan

Who's doing what in the first weeks after baby arrives. Meals, visitor policy, newborn care preferences, feeding approach, emotional support, partner responsibilities. Built for both prenatal planning ("what do we want?") and postpartum execution ("what's actually happening?").

Solo moms see an adapted version without partner-centric language — Cradled handles this automatically based on the client's household setup.

Intake form

A one-time get-to-know-you questionnaire. Medical history, previous births, goals for this pregnancy, known complications, preferred communication style, emergency contacts. Usually assigned at the start of the engagement.

You can assign multiple templates to the same client — they'll all show up on her Care tab.

Custom templates

If the three built-in types don't fit something you need, you can create a Custom template. Same editor, no enforced structure. Useful for things like specialized feeding assessments, follow-up questionnaires, or your own proprietary frameworks.

Using Cradled's starter templates

Cradled ships with starter versions of birth and postpartum plans that you can clone and customize. Look for System templates in the template list. Click Clone to copy one into your own templates, then edit it to match your practice.
